I rarely start new habits. When I do, I rarely stick with them. I periodically start new habits. Sometimes I stick with them, sometimes I don’t. I regularly start new habits. When I do, I’m more likely than not to stick with them. I’m constantly building new habits and sticking with them. Knowledge of Diet: How much do you know about diet? * I know nothing about what a healthy diet should look like. I have a sense of which foods are healthy and unhealthy but don’t know how to put together a healthy diet. I have some familiarity of what a healthy diet looks like. I know about the glycemic index, which foods are high/low in carbs, and what a balanced plate should look like. I have a solid understanding of all aspects of a healthy diet that will be most effective in meeting my specific health needs. Eating Habits: How healthy do you eat? * I don’t pay attention to what I eat. I eat what I want, whenever I want. I am aware that I eat a lot of carbohydrates and processed food. I know I could have a healthier diet. I am trying to limit carbohydrates and processed food but don’t have a plan in place that helps me to be effective. I am eating primarily whole foods and intentially limiting my carbohydrate intake. I am eating exactly as I should in order to achieve my specific health goals. Mindset: How fixed or open is your mindset? * I fail every time I set a goal or try to improve my life. My previous experience leads me to believe I cannot change or grow. I guess there is a possibility that I can learn and make changes to improve my life but I don’t have the confidence to make it happen. I have made positive changes but sometimes don’t believe I can be successful with ongoing change and growth. I have experienced some success and am actively engaged in learning new things. I am confident that I can continue to implement positive changes and growth. I am certain that I am wired with a growth mindset. My life is a consistent effort toward learning and growing. Movement Habits: How much do you move? * I rarely do intentional movement or exercise. I sit for a significant portion of most days. I sometimes think about movement and occasionally do some exercise. I am not at all consistent with my efforts. I try to incorporate movement into my daily routine. I don’t have a clear plan or goals. I have put together a movement plan, schedule it into my days, but don’t follow through consistently. I am dedicated to my movement plan, have clear goals, and feel great with my consistent effort and results. Stress Management: How well do you manage stress? * I am stressed all the time. I have no idea how to manage my stress. I am stressed most days. Most of my stress management strategies are unhealthy. I am stressed sometimes. I am learning healthy stress management strategies but haven’t figured out which ones work for me. I am occasionally stressed. I have determined healthy stress management strategies that work for me and use them regularly. I am rarely stressed. I schedule preventative stress management strategies into my daily routine and consistently use them. Emotions: How is your emotional wellbeing? * I constantly struggle with emotions such as bitterness, resentment, anxiety, depression, and/or others that cause deep concern for my emotional wellbeing. I want to be upbeat but it’s easy for me to slip into negative emotions. Others can sometimes tell I’m struggling but I’m generally not aware of how I’m carrying emotions in my body. I spend about half my time feeling good and the other half trying to avoid and/or recover from time spent in my negative emotions. There are times when I feel negative emotions but for the most part, live in an emotionally healthy space. I can sense my emotional wellbeing in my body. Negative emotions are rare and when they do occur, I recognize and let them go. I am consistently upbeat with a positive and optimistic outlook on life. Sleeping Habits: How well do you sleep? * I don’t sleep well, and feel tired every day. I have no idea how to get better sleep or how much sleep I need. I am aware that my sleep space and current hours of sleep are not ideal for me. I feel tired most days. I have improved my sleep space and know how many hours of sleep I need. I am not consistent with my routine and feel tired sometimes. I have learned what my ideal sleep space and routine should look like. I generally feel rested when I am able to stick with my routine. I have created an ideal sleep space and routine. I am committed to consistency, always wake up rested, and have high energy throughout the day. Mindfulness: How focused and in-control are you? * I am constantly distracted and impulsively reacting to my circumstances. I don’t feel a sense of control over my own life. I don’t believe I can change the way I am. I am often distracted and reacting impulsively to my circumstances. I often lack a sense of control of my own life. I have doubts about my ability to change. Sometimes I am distracted and react to my circumstances without thinking through my options. I sometimes lack a sense of control of my own life. I think I can change, but I haven’t been able to yet. I can usually focus on the present and think through my options before responding to my circumstances. I usually have a sense of control over my own life. I believe I have the ability to change. I tend to be focused and calm, responding to cirmstances in well thought out and healthy ways.
